Diabetic Incentive Program
- Developed to educate our members about the importance of receiving their recommended diabetic tests and exams to stay healthy.
- If the member complies with his or her required testing, pcp /specialist appointments and a dilated eye exam, they are eligible for a wellness incentive after verifying compliance. To encourage compliance with self-care, an additional incentive is given to those members whose HgbA1C is lower than 8% by the end of the year.
Prenatal Incentive Program (Momma & Me Program)
- The "Momma and Me" incentive prenatal program was the first MedStar Family Choice incentive program originally implemented in 1998 for expectant mothers less than 28 weeks' gestation to encourage sufficient prenatal care.
- Once enrolled in the program, the member receives an incentive for the first OB appointment and another incentive for each remaining OB appointment and prenatal education classes through delivery. All appointments are verified with the member's obstetrician. Educational materials are also provided to the member when she enrolls in the program. Attending educational prenatal classes is also encouraged for the expectant Mom to increase awareness of prenatal & postnatal health strategies.
Postpartum Incentive Program (We Care Program) Program Description / Interventions
- Members who participate in this program are eligible to receive an incentive if they receive their postpartum exam 21 to 56 days following delivery and another incentive if their newborn is seen for his or her 2-week Well Child Visit. For keeping both appointments, the member is entered in a monthly raffle for a chance to win another incentive to an area business.
Immunization/Lead Program ( 8-Month / 30-Month-Old Population) Program Description/Interventions
- The Immunization Program targets 8- and 30-month-old children missing their required immunizations and/or lead test. If the member is not up to date, the MedStar Family Choice Outreach Coordinators assigned by age group attempts to contact the parents of our members to educate them about the importance of their child being up to date with their well-child visits and immunizations.
- Assistance with scheduling the member's appointment is also provided. If the parent does not respond to our outreach (telephone / letters), the Local Health Department is notified for assistance.
- A child's first Lead Test at age one is a very important part of keeping the child healthy and preventing lead poisoning. MedStar Family Choice started a new program in 2013, called the MedTox Home Visit Program. This program allows a child member to be tested for lead levels. In addition to making sure the child is in good health, the member will receive an incentive.
Adolescent Incentive Program (12 through 20 Years Old)
- Developed to educate the parents of our teen members and members (>18 years old) about the importance of receiving an annual physical exam and being up to date on immunizations. Parents of our adolescent members less than 13 years old and in need of their meningococcal vaccine and or DTaP/Td vaccine are also educated about the importance of complying with their child's immunization requirements.
- If the member is not up to date, the MedStar Family Choice Outreach Coordinator attempts to contact the parent of our non-compliant member to educate the parent about the importance of their child being up to date with their well-child visits and immunizations.
- For those teen members that comply with their PE, they will receive an incentive.
- Assistance with scheduling the member's appointment is also provided. If the parent does not respond to our outreach attempts (telephone / letters), the Local Health Department is notified for assistance.
Well Child 3-11
- Developed to educate the parents of our 3–11-year-old children about the importance of receiving an annual Well Child Visit and being up to date on immunizations.
- If the member is not up to date, the MedStar Family Choice Outreach Coordinator attempts to contact the parent of our non-compliant member to educate the parent about the importance of their child being up to date with their well-child visits and immunizations.
- Assistance with scheduling the member's appointment is also provided. If the parent does not respond to our outreach attempts (telephone / letters), the Local Health Department is notified for assistance.
- For those parents that comply with getting their child up to date with their well-child visit, they will receive an incentive.
Mammogram/PAP Test Program
- Our Mammogram and Pap Test Program was developed to encourage our members to obtain their annual mammogram and Pap test. The goal is to attempt to contact every female member 50 years of age or older, missing a mammogram or Pap test, and educate the member about the importance of being compliant.
- The Outreach Coordinator attempts to contact every female member, 21 years of age or older in need of a Pap test. If contact is successful, the member is educated about the importance of receiving these services and assisted with scheduling their appointments as needed. If contact is unsuccessful, a minimum of 3 attempts are made via telephonic/mail to contact the member.
- MedStar Family Choice members 21 through 64 years of age who receive a Pap test are entered into drawings three times a year for an additional incentive.
- MedStar Family Choice offers our members 50 years of age or older an incentive for completing their mammogram.
