MedStar Family Choice Transportation Guidelines
First check with your local health department.
Your local Health Department may be able to provide transportation to your medical appointments if:

You have an illness such as a heart or breathing problem.

You have a condition that makes it difficult to ride the bus.

You are not on a bus line.
There is a form that needs to be completed by your primary doctor and faxed to your local Health Department. Contact your Local Health Department for assistance.

Wellness and preventative care transportation
Transportation requests should be made Monday through Friday, between 9 a.m. and 4 p.m., by calling 800-905-1722, option 1. Please have all appointment information (date, time, address and telephone number of doctor’s office) available when calling.
Transportation is provided for primary care, preventive wellness, and chronic condition management visits (specialists).
If it is determined that transportation is being abused or inappropriate behavior occurs, future transportation requests will not be authorized.
Urgent cab transportation
Urgent cab transportation will be provided if you or an eligible family member are ill and require an urgent appointment at your doctor’s office, which will be verified before the cab transportation is approved. An urgent appointment is defined as a same-day appointment, or at the discretion of your doctor.
Cab transportation may be provided for OB doctor visits if you are pregnant.
Please make cab transportation requests as soon as you have your appointment information.
Bus transportation
Bus tokens are sent by regular mail, so requests must be made 5–7 days before transportation is needed.
Tokens will only be provided for routine appointments (doctor appointments, eye appointments). Bus transportation will not be provided for mental health appointments, methadone appointments, or physical, occupational, or speech therapy appointments.
Two tokens are provided per passenger for a full day pass.
Children under 5 years old do not require bus fare.
